Main Tasks: Volunteers will work with patients recently discharged ensuring they have access to provisions and support that is needed to maintain their health and well-being. Tasks could vary from accompanying the patience from hospital to home, ensuring food and health related supplies are available, supporting patients to access relevant benefits, as well empowering people to build their confidence and self-esteem. – Available to attend a local induction – Availability and commitment to attend 4-6-weekly supervision meetings – Comfortable in giving access to contractors and other relevant agencies to service user’s properties. – Carry out one-off-shopping and food bank visits for service user’s – Accompany a patient from hospital to home – Accompany to the cash machine – Supporting in accessing relevant benefits – Working alongside relevant agencies; i.e. social services, discharge teams/departments, care agencies, mental health services, substance misuse services etc – Supporting and empowering people in developing a social network – Overall empowering people in building confidence and self-esteem
